Mercy Care, a not-for-profit health plan and the Regional Behavioral Health Authority for Central Arizona, is conducting online focus groups to support their 2020 Community Needs Assessment.  Your child has the opportunity to participate in an online focus group JUST FOR YOUTH about their thoughts and perceptions of substance use and mental health challenges facing teens.
 
We are asking for your permission to allow your child to participate in an online focus group. After you have read this information, please complete the form below to allow your child to participate.
 
WHAT IS THE FOCUS GROUP ABOUT?
  • Attitudes towards substance use
  • Mental health challenges facing teens
 
WHAT WE ASK FROM YOUR YOUTH?
  • To attend the online focus group for ONE HOUR at the scheduled day/time. All youth will have the choice to participate.
  • Participation is voluntary. They do not have to answer questions they do not want to.
  • To share their ideas or experiences with other teens in the group. We won't use their name in any reports or presentations.

WHY PARTICIPATE?
  • Youth feedback will help community providers and coalitions better understand what is happening in our communities.
  • Youth feedback will guide program planning and support future prevention funding.
 
YOUTH RIGHTS
  • Youth can choose not to participate.
  • Only evaluators from Community Alliance Consulting will be present during the focus group and no recordings will be made.
  • Their answers or participation will not impact any services they receive from Mercy Care providers or programs.
 
YOUTH BENEFITS
  • Youth have the opportunity to give their opinion and help improve programming in their community.
 
YOUTH RISKS
  • Youth should not experience any discomfort by answering the focus group questions.
